    The Role:

    We are recruiting for an enthusiastic and experienced Registered Nurse to join the Residential in Reach Program at Northern Health. Residential in Reach offers speciality health care assessment and a rapid response service for people with complex medical conditions who are living in Residential Aged Care Facilities.

    The Clinical Nurse Consultant role encompasses direct clinical service provision and clinical leadership providing triage, timely assessment, care-planning and clinical intervention to residents residing in Residential Aged Care Facilities in the Northern Health catchment. The successful applicant will be required to work a rotating roster that includes weekend shifts.

    To be successful in this role you will have:

    • Specialist clinical experience at an advanced level in Geriatric Care and a demonstrated ability to deliver responsive, evidence-based care
    • Relevant experience / training in palliative care and advance care planning
    • Experience and good working knowledge of Residential Aged Care environments
    • Proven ability to deliver safe, high quality care and education at an advanced level
    • Demonstrated high-level communication skills
