- Provide high quality and person-centred OT services including assessment and development of comprehensive goal-oriented treatment and care plans, home assessment, equipment prescription and report writing. Demonstrate a range of intervention and therapy strategies in line with goals.
- Complete detailed reports including home modification reports.
- Communicate with consumers and relevant internal and external stakeholders.
- Conduct internal and external referrals to other members of the multidisciplinary team, and external funding providers including SWEP and Geat2Go.
- Essential travel across various aged care planning regions and where operationally needed.
- Tertiary Occupational Therapy qualification with current registration with AHPRA
- Current Victorian driver's license
- Knowledge and experience of Government funded aged care programs including CHSP, HCP, STRC, SWEP Geat2GO and My Aged Care are desirable but not required.

About the role

You will partner with our elderly clients to identify their individualised goals, interventions, and therapies with the aim of providing them and their care givers with support, guidance, and recommendations to promote independence, safety, and positive health outcomes.
