    Requirements

    The Role

    We're on the lookout for an experienced, reliable, dedicated, compassionate and empathetic Support Coordinator who want to make a difference by supporting our participants in reaching their aspirations and leading fulfilling lives.

    What you'll be doing:

    As our newest Support Coordinator at Incluso Services you will be responsible with engaging both internal and external stakeholders, from the first contact with participants, to onboarding and continue with ongoing service delivery. You would need to be able to manage multiple priorities while being able to provide great client service, which is key to your success in this role.

    What you'll bring:

    • Exceptional customer engagement skills and experience, delivering high quality, individually tailored service.
    • A solid understanding of the person-centered approach, the principles of the NDIS and the National Disability Service Standards, with experience working within the NDIS and/or disability sector.
    • Efficiently allocate referrals to ensure prompt access to appropriate services for NDIS participants.
    • Create, review and revise NDIS Service Agreements as needed.
    • Collaborate with NDIS stakeholders to convert enquiries into referrals while maintaining exceptional client service.
    • Work closely with operational team members and leaders to ensure a positive intake and service delivery process.
    • Manage new and existing funding requests to ensure NDIS funds are available for service.
    • Provide high quality and tailored advice to participants who are navigating the NDIS.
    • Conduct face-to-face meetings with clients, including families and stakeholders, as necessary.
    • Ensure robust case management activity with all activity entered into the system, ensuring all progress, plans, communications and interactions with participants and stakeholders are clearly documented.

    What we would like you to have:

    • Established networks within the NDIS and Northern Suburbs Community.
    • Experience in a similar role or previous Level 2 Support Coordinator Experience.
    • Authorised to work in Victoria.
    • Valid Victorian Driver License
    • An understanding of the NDIS, disability, and community services sector
    • Relevant Qualifications: such as Bachelor of Psychology, Certificate IV in Community Services or studying towards relevant qualification will be highly regarded for these positions.
    • Minimum 2-year industry experience
    • Ability to actively navigate and adapt to change.
    • Working with Children Check
    • NDIS Worker Screening Check
    • First Aid/CPR Certificate
