    ABOUT THE ROLE:

    The Commonwealth Home Support Program (CHSP) Occupational Therapy (OT) Team provides a community-based service for elderly individuals living at home and having difficulties with daily activities. We aim to assist our clients to participate in everyday activities, maximise their independence, safety and wellbeing.

    The CHSP AHA will work closely with the OT team to support the delivery of recommendations to optimise or maintain the patient's participation and independence in functional activities and occupations. The AHA will carry out delegated 1:1 follow up, within the community and the clients home; education as well as administrative tasks associated with supporting the OT team. This support ensures effective and efficient service delivery for clients and therapists.

    The CHSP OT team consists of OT's and AHA's working across three teams:

    Western Metro Region: covering the councils of Hobsons Bay, Wyndham, Brimbank, Maribyrnong and Moonee Valley.

    Northern Metro Region: covering the councils of Banyule, Darebin, Hume City, Melbourne, Moreland and Yarra.

    Barwon Region: covering the council of City of Greater Geelong.

    In this role, you will be expected to provide support to clients who live in both the Northern Metro and Western Metro regions while based from the Parkville office. Work vehicles will be available for use.

    Supervision is provided and opportunities for professional development and quality improvement involvement are available. The AHA team actively participates in and contributes to unit activities.

  • Mercy Health – Parkville - Commonwealth Home Support Program - Occupational Therapy service.
  • Part time negotiable; limited tenure until 1/7/2025.
  • Grade 3 AHA with OT experience.
  • Work under the delegation of OTs to support clients to remain safely at home.
  • ABOUT YOU:

  • AHA certificate IV or equivalent.
  • Current drivers licence.
  • Minimum of 2 years demonstrated experience as an AHA, preferably within a community setting.
  • Demonstrated time management and organisational skills.
  • Strong written and communication skills.
  • Understanding of and commitment to customer focused service and outcomes.
