    Established in 1991, Danila Dilba Health Services (DDHS) is a not-for-profit, Aboriginal community-controlled organisation that is committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) region in the Norther Territory.

    We are focused on acute care, immunisation, chronic disease management, women's health, men's health, child and maternal health and sexual health.

    We also provide a range of community programs in areas including health promotion, mental health, social and emotional wellbeing, and alcohol, tobacco, and other drugs.

    WHAT WILL YOU FOCUS ON?

    The Aged Care Team Leader has the primary responsibility to provide leadership, high quality and safe coordination of the Elder Care Support program and DDHS aged care team.

    RESPONSIBILITIES

    Strategic Management

    • Contribute to the development and implementation of strategies to ensure the delivery of an integrated and culturally appropriate suite of relevant Aged Care Services programs that meet the needs of community members.
    • Assist in the identification and implementation of innovations to improve effectiveness and efficiency of service delivery.
    • Effectively lead and manage organisational change, people and budget management.

    Program Delivery

    • Work effectively with DDHS Aged Care staff to support clients, carers, and families through the care planning process to ensure the clients care and supports needs are safely met.
    • Employ a holistic approach to culturally safe, trauma informed care and support to the clients.
    • Provide pre assessment and assessment support to clients including identification of appropriate aged care services and access pathways.
    • Support clients through the aged care process including advocacy to register for MY Aged Care, and the care assessment process.
    • Provide briefing sessions for aged care assessors and provide advocacy for clients through the assessment process including collaboration with assessors through assessment interviews to develop support plans.
    • Dependent on assessed support needs, support clients, carers, and their families to identify and select appropriate aged care service providers.
    • Ensure clients, carers and families are supported to understand service agreements and costs of care and supports.
    • Provide advocacy for clients, carers, and families to ensure client care needs are met in a culturally safe and trauma informed way. When required advocate on behalf of clients, carers, and families to address any issues with service providers.
    • Provide support to Carers and Families to understand the clients care and support needs which may include linking carers and families to other related support services, transfer of care and support and information relating to care and support options and costs.
    • Build internal and external relationships with service providers, organisations, and assessment bodies so that informed choices can be provided to clients.

    • Actively model DDHS' Code of Conduct Policy and demonstrate appropriate and professional workplace behaviours, help team members as required and undertake other key responsibilities or activities as reasonably directed.

    • Maintain a positive work environment along with harmonious and cooperative relationships with all employees, and clients.
    • Develop and maintain effective relationships internally and externally.
    • Represent DDHS in a variety of settings ensuring the organisation's brand is protected and enhanced.
    • Maintain professional work ethics and participate in staff meetings.

    Workplace Health and Safety

    • Conduct all activities in a manner consistent with national health accreditation standards, DDHS Occupational (Work) Health and Safety Policy and procedures.
    • Proactively address WH&S hazards, incidents, and injuries.

    Quality

    • Identify and make recommendations on opportunities to improve processes, quality, and service delivery outcomes.
    • Participate in the development of quality procedures and contribute to internal and external program reviews as required.
    • Implement/follow the DDHS incident and complaint investigation policies.
    • Comply with DDHS Privacy and Confidentiality of Health Information Policy.
    • Ensure data, reporting and compliance obligations for services are met.
    • Organisational Responsibilities

    • Other duties as required, consistent with skills and experience, as directed by the Supervisor.
    • Qualifications in a health-related discipline- and/or associated significant experience.
    • Demonstrated experience in a health or care sector role working with older Aboriginal and Torres Strait Islander people.
    • Proven ability to lead and work within a Team environment that is open and transparent, with a person-centred focus.
    • Experience working with Aboriginal and Torres Strait Islander people.
    • Excellent communication and interpersonal skills, with the ability to build trust with community.
    • Demonstrated understanding of the principles of person-centred care.
    • Demonstrated understanding of culturally safe and trauma informed care.
    • Demonstrated understanding of the aged care system with the ability to identify, assist and support clients to access services and reduce service complexity and confusion for clients and constructively negotiate outcomes with a diverse range of people and service providers.
    • Sound understanding of My Aged Care, and the Aged Care System.
    • Ability to work autonomously and be a self-starter, with demonstrated high initiative and problem-solving skills.
    • Tertiary qualification in a health or aged care discipline
    • AHPRA registration or eligibility for membership of a relevant Professional body.
