    Job Description:

    • Full time, permanent Grade 4 Mental Health Nurse position available in a Mental Health Community role.

    About the Role

    As an Aged Mental Health Clinician, you will work as part of the community aged mental health team providing comprehensive psychiatric assessment and treatment to clients at home. You will be responsible for intake and triage, crisis support, assessment and case management, reviewing risk and providing support to reduce the impact of illness and enhance independence to our clients and carers in various community and residential settings.

    You will provide discipline-specific support and advice to the team, utilizing a cooperative and recovery-focused approach across the continuum of care to achieve optimal outcomes for consumers.

    The successful candidate will have the opportunity to work with a diverse range of clients and be part of a cohesive, supportive team committed to the wellbeing of colleagues and consumers.

    Key Responsibilities

    • Provide comprehensive psychiatric assessment and treatment to clients at home.
    • Conduct intake and triage, crisis support, assessment and case management.
    • Review risk and provide support to reduce the impact of illness and enhance independence.
    • Collaborate with the team to achieve optimal outcomes for consumers.

    Requirements

    • Postgraduate Diploma in Mental Health Nursing, or a specialist undergraduate Mental Health Nursing program, or a specialist post-basic course of training leading to registration as a Division 3 Nurse.
    • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
    • 5 years' post-qualification clinical experience within Mental Health and/or aged care clinical services, including case management and care coordination experience.
    • Demonstrated passion for achieving the best clinical practice.
    • Relevant knowledge of the Mental Health and Wellbeing Act 2022.
    • A valid Nationally Coordinated Criminal History Check, or willingness to obtain.
