    Join our team as a Case Manager for Victim Survivors at Primary Care Connect There are multiple positions available, both fixed term and ongoing.

    About the Role

    This Case Manager for Victim Survivors works within the Family Violence Team with the aim to provide services to those who have experienced or are affected by family violence or those that are at risk of being unsafe in the family environment. The target group includes those for whom family violence contributes to circumstances that threaten, or adversely affect the adequacy, safety, security and affordability of housing and maintenance of the family environment. This target group also includes those who are currently in an abusive relationship and those who have not previously accessed police or crisis services for family violence related matters.

    The Case Manager for Victim Survivors will provide education, information, advocacy and referrals within the required local, state and national guidelines and frameworks. The service areas for this position include Greater Shepparton and Moira.

    Key Selection Criteria

    The following criteria must be met and addressed in applications for consideration for this position:

    • Bachelor in Social Work or equivalent, a willingness to work towards an equivalent qualification as set out in Mandatory Minimum Qualifications - Qualifications Mapping
    • Previously participated in or a willingness to undertake MARAM Framework training
    • Experience in providing assessments, care planning, service coordination and writing case notes and reports.
    • Experience in providing phone and face to face Case Management
    • Knowledge of the principles relating to confidentiality and duty of care
    • Experience using technology to complete daily work tasks.
