- Support young people in line with Youth Services model of care and associated Frameworks
- Build effective relationships with clients that encourage self-determination and participation
- Provide ongoing assessment of client's issues, safety, needs and circumstances from initial intake to exit
- Develop individual case plans in collaboration with clients including a range of interventions that address the clients needs
- Assist clients to access support services and establish community supports including referrals to appropriate specialised agencies
- Maintain accurate up to date client records including case notes using TSA information systems
- Degree qualification in social work, welfare, community development or related field
- Demonstrated high level of sector knowledge and skills attained through previous roles, service and study/development
- A valid Employee Working with Children Check
- Psychological Assessment (in line with Children and Young People Safety Act 2017
- Through their Eyes (Reporting Abuse and Neglect) Training
- A First Aid Certificate
- A willingness to undergo a National Police check

Why this role is important
We are looking for a passionate, relationship focused Case Manager to join our Youth stream This is an incredible opportunity to be part of a dynamic team that has a client focus whilst supporting each other as a team.
In this role you will provide responsive case management to young people between the ages of 15 and 25 years of age, while effectively building relationships with clients and encouraging self-determination and participation. As well as that, you will be supporting the development and growth of young people's lives through the collaboration with internal and external stakeholders.
