- Ensure appropriate and holistic services are provided to meet the multidimensional needs of older people and their support networks.
- The ability to review and document client progress while on the program via regular phone contact, consultation appointments, or home visits
- Help to remove barriers affecting clients' ability to engage in their community.
- Develop culturally appropriate models of service delivery.
- Advocate for service provision in areas of unmet need.
- Assist clients with adjustment to life changes and other transitions.
- A degree in Social Work and a membership with the Australian Association of Social Workers (AASW) or International Federation of Social Workers (IASW)
- Clinical and practical social work knowledge
- Effective communication and counselling skills to support client needs, motivation, and ability to engage in treatment.
- Strong interpersonal and communication skills enabling effective interaction with clients and families, team members, health professionals, and stakeholders
- Experience providing evidence-based assessment and planning for clients, focused on reablement and independence using a wellness and restorative approach.
- The ability to work autonomously and as a member of a multidisciplinary team
- A current driver's licence and access to a reliable vehicle
- Experience facilitating education for clients, community groups, or organisations.
- Comprehensive understanding of community-based service provision
- An understanding of risk management, health and safety, and continuous quality improvement processes
- The ability to evaluate and modify the effectiveness of social work programs/interventions
