    The Transition Care Program is seeking an experienced, innovative and flexible individual to work as a member of the TCP Physiotherapy team.

    The program is a jointly funded program (DHS/Commonwealth), and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long-term care arrangements.

    TCP operates in both the Community as well as two Residential Aged Care environments, offering scope for clinicians to enhance their experience along the care continuum.


    The successful applicant will need to be flexible in terms of working across both residential and community areas.

    The Physiotherapist will need to demonstrate sound clinical judgement, assessment and practice skills, and have an understanding of the issues affecting patients with complex functional and cognitive needs and an interest in aged care issues.

    The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.

    What You Need


    We anticipate you will have:

    • Current registration with the Physiotherapy Board of Australia (through AHPRA).
    • Relevant experience as a Physiotherapist in the subacute/ GEM setting with sound understanding of aged care issues and processes.
    • Demonstrated clinical leadership and supervision of students to promote professional competence and foster individual development.
    • Demonstrated ability to coordinate and participate in quality improvement and evidence based practice activities.
    • Excellent communication and interpersonal skills, including negotiation, conflict resolution and problem solving.
