About the role
We are looking for a Nurse Home Visitor to join our team in theFamily Partnership Program (FPP) at Congress.
The Australian Nurse Family Partnership Program (FPP) is anational nurse home visitation program that has been rigorouslyevaluated for over 25 years.
The program involves Registered Nurses and/or Midwives workingin partnership with Aboriginal Family Partnership Workers andenrolled clients who are pregnant with an Aboriginal baby fromearly in their pregnancy and continues until the child is 2 yearsold, through an intensive culturally responsive home visitingprogram.
The Congress Nurse Home Visitor and Aboriginal FamilyPartnership Workers develop a long-term relationship with clientsthat builds client capacity, skills, confidence to ensureself-determination with the ability to determine their ownfuture.
