    Lead Reablement Worker Role

    This role is a vital part of our care at home service, designed to help individuals maximise their independence.

    • The successful candidate will play a key role in supporting people as they transition towards independent living, working closely with Occupational Therapy teams and other health professionals to set and monitor personalised goals.

    Main Responsibilities:

    1. To be the primary point of contact for service users, developing strong relationships built on trust and respect.
    2. To create tailored reablement plans that focus on recovery and independence by identifying areas where support can be provided most effectively. To develop strategies to empower service users to regain control over their daily lives. Develops and implements comprehensive plans promoting physical activity , education about safe practice lifestyle changes which contribute greatly toward overall well-being . Maintains patient records including demographic information about gender identity race or ethnicity language spoken when appropriate supports proactive approach maintained through continuous assessment from medical providers via available resources communicated during interactions Additionally ensures establishment closure procedures appropriately managed final report detailing successes challenges recommendations potential alternatives based on actual progress since case assignment < li > Collaborates with multidisciplinary team members namely (Occupational Therapists Speech-Language Pathologists Physiotherapist Social Workers) engaging patients families caregivers navigating conversations regarding best options whether short term long duration optimizing growth maintaining momentum leads educational workshops focused learning skills everyday practical tasks life outside hospital setting integrating therapeutic exercises strategies along similar concepts addressing underlying issues presented verbally prior solution garnered communication facilitating successful participation smooth transitions directly aforementioned innovative techniques suitable targeted interventions providing empowerment feel confident carry out responsibilities making conscious choices believe ability increase helping own abilities/made difference small person greatly improving my quality regardless.
