    An exciting position of Registered Nurse - Clinical Care Coordinator has become available for a motivated and passionate individual wanting to further their career.

    *About the Role
    • Create and implement clinical care plans.
    • Attend to and provide clinical care for our clients.
    • Organise and facilitate all allied health services for our clients.
    • Liaise with care workers and providing training when necessary.
    • Overseeing and implementing quality improvement initiatives to ensure high quality of services.
    • Promote a strong customer focused culture.
    • Ensure all documentation, procedures and reporting meets relevant guidelines and legislation.
    • Ensure compliance with relevant legislative requirements.
    • Maintain up to date, clear and accurate client care plans, records and meet administrative requirements in accordance with policies, procedures and funding guidelines.
    *Requirements:

    • Current AHPRA Nursing registration.
    • Driver's license and access to own vehicle.
    • Professional verbal and written communication skills.
    • High level of organisational skills.
    • Sound computer skills in Office and Excel (or similar).
    • Full Australian working rights.
    *A successful candidate will have the following:
    • Ability to provide exceptional support to our carers when they need assistance with anything related to our external clients - Empathy is essential
    • A passion for respectfully empowering and caring for people.
    • Excellent written and verbal skills as you will have to communicate with clients and our team.
    • Able to work as part of a team.
    • High level of time management and organisational ability.
    • Willingness to participate in further training and education opportunities.
