CalvaryPermanent Full Time,Permanent Part Time About the roleAs the Case Manager at Calvary Community Care, you will be responsible for delivering case management to our clients that is person centred and enablement focused. You will be involved in all aspects of care coordination, including administrative tasks and rostering of client services. You will develop positive working relationships within the local service centre, and more broadly across the organisation to facilitate improved outcomes for our clients.In this role, you will act in accordance with Calvary Community Care policies and procedures, program guidelines, the National Standards of Practice for Case Management and the National Code of Ethics and CCC's Case Management Model.We are currently seeking experienced case managers to join our team in Melbourne on a permanent basis. Full and part time opportunities available. About YouWith experience in Case Management, you pride your self on delivering quality, client focused care. Your ability to build strong relationships is second to none, and you are known for your knowledge of various funding sources in the aged care sector.
