To help maintain and grow this standard, we are seeking an experienced Care Coordinator particularly with experience in the National Disability Insurance Scheme, Aged Care, Community Nursing and or Home Care Sector to provide high quality services and oversee our day to day operational activities. As an ideal candidate, you will have a passion for providing community-based care underpinned by robust quality standards. The role includes an element of attending shifts with clients to ensure you keep current on in home care processes and procedures and to enable you perform quality checks on the standard of care we provide to our community s most vulnerable.Key to the role is an ability to successfully develop, maintain, and build positive relationships with our internal carers, participants patients, their families, and external parties. The Care Coordinator is responsible for creating and refining policies and procedures in line with regulatory requirements, as well as overseeing the progress, and review of service agreements and billing and supervision of staff associated with these streams. The successful applicant will possess highly developed people skills they will be motivated, compassionate and able to make this role their own by utilising their demonstrated work experience in the disability, nursing, or human services sector.Reporting into our Operations Manager who is committed to the development of their people, you will lead and be part of the client services team where you will be required to deliver outcomes against a designated caseload. You will also be responsible for supporting a multi-disciplinary team of professionals who are looking after NDIS Participants and other community clients.
