This casual position is approximately 21 hours per week. The successful applicant will be responsible for conducting in-home assessments, developing and evaluating support plans to meet the needs of the individual Participant . They will also liaise with other health professionals and multi-disciplinary teams in order to meet service delivery goals. There is a requirement for the Community Coordinator to work closely with Support Workers for each Program to ensure that they have the required skill levels to support the Participants. Critical to the success of this role is the ability to communicate effectively verbal written with a diverse group of people with the focus on service. Essential Requirements Endorsed Enrolled Nurse EEN with current AHPRA registration Ability to work independently with effective communication, initiative, problem solving and time management skills Valid QLD Driver s License and reliable vehicle Australian Resident Electronic records Computer Skills to support administrative processing requirements NDIS Check The Benefits The successful applicant will be provided with a detailed orientation process and development program to support the growth of the skill set and be part of an ongoing Peer Support Program.
