Working as part of our multidisciplinary healthcare team, you’ll coordinate and support the delivery of home-based health care across Australia. Your clinical environment will be driven by human-centred, holistic, evidence-based care and supported by generous professional development.
What you’ll be doing
Acting as the central point of contact for clients Implementing, monitoring and modifying in-home care plans to meet client needs Supporting clients to achieve their personal and clinical goals within their home environment Responding to enquiries from clients, referrers, service providers and funders Supporting the medical administrative team to allocate service providers and complete records and documents Helping to improve the health and wellbeing of hundreds of Australians.

Your skills and experience
Qualification in occupational therapy or physiotherapy with current AHPRA registration Experience working in a multidisciplinary collaborative team Excellent knowledge of hospital discharge planning Ability to work in a dynamic environment with tight timeframes and competing priorities Excellent clinical reasoning and problem-solving skills Excellent verbal and written communications Strong administration and time management skills, tech savvy Passionate about health and committed to making a real difference in the community.
