The Bendigo Bridge Alcohol and Other Drugs Residential Program offers a 18-bed residential program for men & women to address issues caused by Alcohol & other drugs. A combination of individual case management and support, group work, life skills, self-awareness and staff role modelling are used to encourage participants to make positive changes. Individuals are supported to:
• Develop appropriate life skills & resources
• Develop the right support from their established or newly developed support mechanisms
• Develop the necessary support services to ensure a recovery-oriented treatment journey
The AOD case manager position is responsible for supporting clients from admission through to completion, focusing on developing a holistic treatment plan that includes the:
• Identification of a Post exit plan
• Implementation of relevant referrals are made to support with:
• Finances, Family & relationships, Offending, Mental Health , Physical health, Meaningful use of time, Community
And Drug & Alcohol Use.
The AOD case manager position also is required to Deliver therapeutic and educational groups and conduct case management sessions a with up to 6 clients a minimum of once per week per clientCase management practice, including assessment, monitoring, planning and advocacy for the client is responsive, client focused and reflective of contemporary theory and practice.Collaborate with and support clients to develop Individual Care Plans and Treatment Goals based on ongoing assessmentWork with a trauma informed practice lens that is individual focused, and embed in the team’s case management practiceLiaison, advocacy and networking with relevant services that is focused on case management practice and improving client outcomes.Client progress and outcomes are monitored, in conjunction with the Senior Case ClinicianLanguage used in client work is inclusive and feedback encouragedUnderstanding of and commitment to the mission and values of The Salvation Army.Compassion and professionalism in all actions and ethicsExtraordinary service to customersOutstanding quality in everything we doGrowth, development and safety of people Relevant experience in a social service environment and / or AOD sectorExperience working with a trauma informed practice lensExperience in the provision of case management practiceSound experience of working with people from diverse backgrounds and with a complex range of support needs.
