What you'll be doing
The Integrated and Community Health Chronic and Complex Service provides a range of high quality preventative, early intervention and treatment services for clients with chronic andor complex health issues.

The Chronic and Complex Service Coordinator is responsible for the day to day operational management of the Allied Health multidisciplinary teams based in Community Health including the disciplines of Occupational Therapy, Physiotherapy, Podiatry, Speech Pathology, Social Work and Dietetics. The position is responsible for the management of Aged Day Service Nurse Unit Manager.

Selection Criteria:

  1. Relevant clinical qualifications in a health related field and extensive recent clinical experience andor experience in the management and planning of health services
  3. Substantial, recent experience in the working in management of multidisciplinary clinical teams and effective staff management and clinical leadership skills.
  5. High level communication and analytical skills including proven experience in the management of clinical activity and performance monitoring.
  7. Proven knowledge and experience of primary care and community health clinical service delivery and clinical practice models and proven skills to efficiently plan and manage clinical services.
  9. Understanding of Integrated health & chronic and complex health policies and strategic frameworks.
  11. Proven organisational and planning ability
  13. Knowledge and experience of quality processes and a commitment to the implementation of best practice and clinical service innovation.
  15. Currently hold a drivers licence valid in Australia
