Are you an experienced, passionate NDIS Support Coordination professional who is passionate about supporting people living in remote communities, super-comfortable with technology and willing to travel?
We are seeking an experienced NDIS Support Coordinator to do FIFO work travelling to Country and Outback Queensland on a 70-hour fortnight, 7:7 roster.
We are an NDIS registered provider for Specialist and non-specialist Support Coordination. We provide NDIS Support Coordination services to remote communities where there are few or no other providers. This thin market service delivery environment necessitates an organisational aspiration to be a best practice provider to ensure remote communities can access equitable NDIS services on parity to the larger cities.
We build teams of Lifestyle Support Workers, Coordinators and Allied Health professionals who work together to ensure we put the person and their family at the centre of our practice. You will work closely with both Lifestyle Support Workers and Allied Health multi-disciplinary professionals to find creative solutions to ensure that people in remote communities have access to NDIS service. Stream Services collaborates closely with other remote service providers as well as the NDIS Thin Market and Remote Planning Teams to find unique solutions for NDIS participants who have unique barriers to receiving services because of their isolated location.
At Stream Services, we are committed and focused on upskilling our workforce and professional development. NDIS Support Coordinators may be eligible for a development program for Specialist Support Coordination which will include Support Coronation specific training as well as accredited training with Central Queensland University to complete over a 10 month period a Certificate IV in Disability or a Diploma in Community Services while working with remote and very-remote participants.
Current Blue and Yellow Card (or eligible to obtain)
If you have lived outside of Queensland within the past five years; a recent National Police Check certificate.
Completion Certificate of the NDIS Quality and Safeguards Commission Worker Orientation Module.
Must have at least one year experience as an NDIS Support Coordinator and a minimum of 5 years NDIS or disability industry experience.
Demonstrate an understanding of and ability to implement and maintain appropriate documentary evidence of compliance with the Specialists Support Coordination Practice Standards certification module (pages 36-37 https://www.ndiscommission.gov.au/document/986).
Previous qualifications in psychology, occupational therapy, social work or other allied health, developmental educator, social or health science.
