About the Child and Youth Community Health Services
The Child and Youth Community Health Services (CYCHS) brings together a variety of specialist community services to help our kids and their families to lead healthier lives.
CYCHS predominantly provides front line health care to communities throughout south east Queensland. In fact, the service provides access to community care to around , children, which represents per cent of Queensland's children. A range of services are provided across the continuum of care as well as health promotion.
A number of Child and Youth Community Health programs, including Deadly Ears, reach communities throughout the entire state.
The service's (full time equivalent) staff work in and from more than community health centres, plus clinics, hospitals, schools, shops and offices, covering a geographic area beyond Beaudesert in the south to Kilcoy in the north.
The Clinical Nurse (CN) role provides advanced clinical and problem-solving skills, expert planning and coordination skills in the clinical management of clients with complex care needs, ensuring quality standards are met. This role works autonomously within the Healthcare Team structure and is pivotal in the planning, managing and delivering of specialist nursing care. The CN provides clinical leadership for professional and clinical practice, education and research. The role supports in management activities including portfolio responsibilities.
The Child Health Service (CHS) provides multidisciplinary primary community care services for children who reside in the CHQ HHS geographical area. The CHS provides a child centred, family focused health service, which includes promotion of health and development, early detection, early intervention, and care coordination. This is achieved through enhancing opportunities for families to access support, developing linkages with community groups, building confidence and capability, and enhancing relationships between children and parents.
The CHS is underpinned by a Model of Care which provides the framework of an evidence-based, clinically safe practice and efficient use of resources. Clinical services are provided by qualified, culturally competent healthcare professionals, with specialist qualifications and/or experience in children's health and include Advanced Health Workers, Child Health Nurses, Social Workers and Psychologists.
