    Job Description

    Achieve Beyond is seeking a dedicated Service Coordinator to join our growing Melville, Long Island team. The successful candidate will oversee the process of assisting families enrolled in the Early Intervention Program (ages birth to 3) by combining traditional case management activities with family-centered care principles.

    Key responsibilities include:

    • Oversight and management of an Early Intervention caseload of 45-55 cases.
    • Timely follow-up on cases upon assignment.
    • Maintaining ongoing services and therapist performance for children receiving Special Education, Speech Therapy, Physical Therapy, Occupational Therapy, and Autism Services.
    • Assisting families in identifying available evaluation or service providers.
    • Independently managing paperwork submissions and implementing NYS and NYC Regulations and Policy.
    • Educating families about advocacy services.
    • Coordinating services with medical and health care providers.
    • Facilitating transition plans to preschool and other services as necessary.
    • Navigating web-based portals and customized databases with minimal supervision to update information and documents.
    • Conducting field work with families, including home visits and office meetings.
    • Documenting and submitting activity reports daily with accurate information.
    • Attending monthly Supervision meetings for training and development.

    Requirements

    • Bachelor's degree in a health or human service field.
    • Strong computer skills and Microsoft Office knowledge.
    • Self-motivated individual able to work independently.
    • Excellent organizational and communication skills.
    • Bilingual Spanish/English proficiency.
    • Ability to multi-task and meet multiple deadlines.
    • Set-up to work from home when needed.
    • Willingness to travel around boroughs for face-to-face contact.

    Physical Requirements:
