    Job Title: Support & Advocacy Case Manager - Housing and Homelessness

    This is a 6 month contract position with an ASAP start, working at SCHADS level 5.

    We are seeking an experienced and motivated Case Manager to provide holistic, trauma-informed, client-centred support through case management, advocacy, and collaboration with stakeholders.

    Duties:

    • Deliver tailored case management plans to meet diverse client needs, taking into account individual circumstances and goals.
    • Work collaboratively with families and individuals to provide practical support, advice, and advocacy, acknowledging their strengths and abilities.
    • Foster strong partnerships with service providers and stakeholders to optimise outcomes and ensure clients receive comprehensive support.
    • Maintain accurate case notes and reporting to meet program objectives, tracking progress and identifying areas for improvement.
    • Supervise and support volunteers or placement students as needed, providing guidance and mentorship to help them develop their skills.

    Requirements:

    • A degree in human services or a related field.
    • Experience working with individuals and families experiencing complex challenges such as mental health issues, domestic violence, financial stress, or barriers to education/employment.
    • Strong interpersonal and communication skills, with a commitment to social justice and inclusion, enabling effective relationships with clients, colleagues, and external stakeholders.
    • Knowledge of trauma-informed, person-centred case management practices, understanding the importance of sensitivity, empathy, and respect in supporting vulnerable individuals.
    • A current driver's licence, Blue Card, and current police check.
