About the Role:
We're in search of a dedicated and experienced Permanent Care Manager with a genuine passion for person-centered care. As part of our esteemed team, you'll oversee the well-being of residents in a prestigious 96-bed aged care facility located in Queensland's Fraser Coast Region.
Key Responsibilities:
– Ensure residents receive safe, high-quality care consistent with our organisation's model & philosophy.
– Manage assessment, care planning, and evaluation efficiently.– Foster the development, supervision, and support of the care team.
Selection Criteria:
– Minimum 2 years' experience in care management within residential aged care.
– Demonstrated passion for delivering person-centered care.
– Exceptional communication and interpersonal skills.
– AHPRA registration and NDIS Worker Checks are mandatory.
