    About the department

    The department provides a comprehensive range of kidney care services to inpatients and outpatients with kidney disease. The renal inpatient ward is based on Ward 7 North and is supported by 7 North Dialysis, both situated in the Austin Tower. The Department of Nephrology operates a comprehensive patient-centred dialysis program including home haemodialysis, peritoneal dialysis, and facility haemodialysis. The department is responsible for a full range of outpatient renal clinics, with more than 8,000 attendances per year, including renal medicine, renal transplant, dialysis, live donor clinic and renal genetics.

    About the role

    The Chronic Kidney Disease (CKD) clinical nurse consultant is responsible for the management of patients with stage 4 and stage 5 chronic kidney disease from referral to the nephrology service through to receiving a kidney transplant, commencement of dialysis, or palliative care. The CKD clinical nurse consultant undertakes patient assessment, planning and education, ongoing monitoring and facilitates patient-centred care applying a shared decision-making approach, and ensures appropriate and timely referrals.
