    Description
    War Memorial Hospital (WMH) - Aged Care Assessment Team
    • Location: Waverley, South Eastern Sydney
    • Services: 24-hour inpatient care, outpatient departments, specialist community teams
    • Role: Join our multidisciplinary ACAT as a Nursing or Allied Health professional
    About the Role:
    • Undertake comprehensive assessments of older individuals in various settings
    • Mobile role covering multiple areas within the district
    • Assess physical, medical, cognitive, psychological, environmental, and social aspects
    • Provide assessment services to older and younger individuals with age-related needs
    Requirements:
    • Tertiary qualifications in Nursing or Allied Health
    • Minimum 2 years post-graduate experience with older people
    • Sound knowledge of healthcare needs in various settings
    • Demonstrated care planning and communication skills
    • Ability to work autonomously and in a team
    • Knowledge of Quality Improvement initiatives and Work Health Safety principles
    • Current unrestricted driver's license in NSW
